Form DL396B

Background: This form is very simple and pretty much self explanatory. State law requires that any Driver’s Education and Driver’s Training programs should be taught in a manner that is “educationally equivalent” to that of the state. See CDE section 51220.6. This form asks several yes/no questions that certify that what your private school is teaching is “educationally equivalent” to that of the state’s.


Complete the name and address portion in the same manner as DL396 was completed. Be sure and put your K# next to the school name in the school name box on the form. It doesn’t ask for it, but you need to have it on every piece of paper that is returned to the DMV.


Check “yes” for every box on both pages. You must teach these topics in order to be educationally equivalent to the state. This form is a very good outline for your course that you will teach your students. You can use it for the backbone of your lessons.

Sign and date the form on the second page. Make sure that you print the signer’s name next to or below the signature.
