for Completing Department of Motor Vehicles
Form DL396A ~ Driver Education Supply Request
Background: This is the form that you will use each year to order the forms
necessary to teach your students DE/DT. Only three of the forms are crucial,
but it is very helpful to have several others that are offered here. Many of
the forms are readily available at your local DMV office. Each one will be explained
here, and its importance will be noted.
The regulated forms are limited in the amount you can request. You may request one per student, plus 1% for mistakes. So if you have one student, you may request 1 for your student, and 1 for making mistakes. If you are teaching less than ten students, then you may only order one more than the number of students you have in your class. Remember that you may only teach students who are registered as full time students in your private school.
You may order any number of non-regulated forms that you wish. There are two columns on the form, one to list the amount of each form you have on hand, and the second is to request additional forms. I put a zero in boxes where I am not ordering or do not have any forms on hand.
Be sure to sign and Date this form and keep a copy of it for your records.

DL-5B. Student
Exam. Order one for each student plus one or two for the teacher. This also
comes with a correction form. This is very handy to have for your students to
practice before they go to the DMV to take their actual tests.

DL-5BS. Spanish
Student Exam. Handy to have if your child is learning Spanish. You may also
write a separate request form if your student needs another language for the
exam. Check the website, several are offered.

DL-38. Statement
of Facts and certification of Insurance Coverage. You don’t need this
form. It is used by schools to assure them that each parent has automobile insurance
on their own vehicles and that their student driver child is covered by that
insurance. You do not need to certify to yourself that your child is covered
by your insurance policy, but you do need to check with your insurance agent
to be sure that your child is covered. Some insurance companies require for
them to be listed specifically on the policy once a permit is obtained. Other
insurance companies require it only after the child is licensed. It is best
to check before you begin teaching the class, so that you will have time to
decide if you want to change companies if your company charges permitted drivers.
Most do not charge, and I have found that the ones that do, charge excessive
rates for both permitted and newly licensed drivers.

DL-44 Application
for License. These are handy to have, so order a couple. They are also readily
available at every DMV office in several languages.

DL-118 Student
License. This form is not technically necessary, for a private school student
who is taught by his parent/teacher. The form is to be issued upon consent of
the parent and will prove that the child has approval from the school and the
parents to be behind the wheel. As a home school family who files the PSA, you
will be teaching your own child. Your child will have a permit issued by the
DMV before he can begin DT. This permit is valid certification that he has completed
the proper coursework and passed the exam at the DMV office. The student license
is not necessary.

DL-119. Statement
of consent and Acceptance of Liability. This form is also not necessary for
a home school family who will be teaching their student/children for the same
reason listed for DL-118 above.

DL-387. REGULATED
FORM. Certificate of completion of Driver Education. This is the form you will
issue when your student has completed the thirty hours of classroom study for
Driver’s Education. You may order 1 per student plus 1% for spoilage.
You must safely guard and keep track of this form. You will need to write the
registration number on the DL-803 when you issue it to your students.

DL-388. REGULATED
FORM. Certificate of completion of Both Driver Education and Driver Training.
DO NOT ORDER THIS FORM. It only confuses the clerks at the DMV office since
schools do not generally use it, and it is not necessary for you to use it for
your student. Several years ago, this form was split into two separate forms,
the DL-387 and the DL-388A. You will use those forms for your students.

DL-388A. REGULATED
FORM. Certificate of completion of Driver Training. This is the form you will
issue when your student has completed the six hours of behind the wheel training.
You may order 1 per student plus 1% for spoilage. You must safely guard and
keep track of this form. You will need to write the registration number on the
DL-803 when you issue it to your students.

DL-392. REGULATED
FORM. Certificate of Enrollment in Driver Training. This is the form you will
issue when your student has completed the thirty hours of training for Driver’s
Education. It will be given to the DMV when they take their exam for their permit.
It tells the DMV that the student is enrolled in a Driver’s Training program.
You may order 1 per student plus 1% for spoilage. You must safely guard and
keep track of this form. You will need to write the registration number on the
DL-803 when you issue it to your students

DL-600 and DL-600S.
California Driver Handbook – English or Spanish. These are available at
every DMV office, but you may order them here also. Order one per student plus
one for each teacher.

DL-603. Parent-Teen
Handbook. Another handy booklet to have at the beginning of your course. Order
one for each student and one for each teacher. You will probably be given one
when your student passes the written exam.

DL-665. Motorcycle
Handbook. This is a crucial book to have, even though your students won’t
be riding motorcycles. They must learn about the rules and safety measures needed
for motorcycle drivers. This is a good way to teach them.

DL-612. Order
one for each school. This book will help you complete the above forms.

DL-803. DE/DT
completion certificate log. Order 1. You will need it to record the issuance
of all the regulated documents that the DMV will give you. This form is also
available online.
